Definition:
The Long Term Care (LTC) data is administrative health data containing records of admission and discharge, assessments, level of care and chronic and rehabilitative services provided in Manitoba at long term care hospitals and personal care homes (including patients awaiting placement). For Winnipeg, additional data is available on screening, clinical and functional status, and Activities of Daily Living (ADL) assessments for residents living in personal care homes. The Long Term Care (LTC) Data consists of two groups of data:

Long Term Care (LTC) Utilization History Data,
and
- Long Term Care (LTC) MDS (Minimum Data Set) Assessment Data.
