Term: Chronic Disease Score

The Chronic Disease Score is a measure of comorbidity based on the aggregate number of prescription medications for a patient. The index contains 24 categories and is based on the Anatomical Therapeutic Chemical (ATC) codes of medications used in the treatment of different chronic diseases.
