Definition:

An event used to describe the outcome of hospital discharge (see Separation(s)), including: 1). discharge home; 2). discharge to nursing home (see Personal Care Home (PCH)); 3). died in hospital; or 4). transfer to other institution (acute or chronic care hospital).
