Definition:

The six-month assessment used in the Insight Mentoring Program is designed to be completed by each participant's mentor every six months based on their knowledge of the participant, to track how she is progressing through the program. The questions on this form cover similar life areas as the Addiction Severity Index (ASI); however, questions are not phrased in precisely the same manner. Areas covered include: alcohol and drug treatment, time spent with mentor, abstinence from alcohol and drugs, risk behaviours, family planning, connection to community services, and family stability such as changes in housing, income, social support, child custody, involvement with the justice system, and education.
