Concept: Data Sources / Indicators Related to Maternal / Child Health and Early Childhood

Introduction

This concept identifies all of the health, social, justice and education data sources available in the Manitoba Population Research Data Repository (Repository) at MCHP that could be used for research into maternal / child health and early childhood. The concept also includes a list of selected variables from each data source that highlights what is available for this type of research. For purposes of this concept, the data sources and variables are divided into four time periods, focusing on prenatal, birth, postnatal to early childhood, and at any time during these periods.

The first section of this concept lists the most relevant data sources in the MCHP Data Repository for the periods of prenatal, birth and postnatal to early childhood, and then additional data sets that could be relevant at any time, depending on circumstances of the newborn/child and mother. This includes data from the areas of health, social, justice and education held in the MCP Repository.

The second section of this concept provides detailed information about each relevant data source in each period of time by providing:


• a link to the corresponding data description available on MCHP's external web site that contains a brief summary of the data and information on the source agency, purpose, years available, type, size, data level, components, access requirements and more;
  
• a brief description of what the data set contains;
  
• data highlights - a list of relevant variables that are available in the data; and
  
• links to additional information in the MCHP Concept Dictionary / Glossary that further describe the data, if available.

The last section of this concept identifies additional MCHP concepts that list and provide access to more detailed methodological information on common health status indicators used to measure maternal, newborn and child health in MCHP research. Links to these concepts are provided.

Data Sources by Timeframe

There are over 30 data sources identified in this concept. Some of these data sources focus on specific events that may occur during the prenatal, birth and postnatal to early childhood periods. Additional data sources that are listed focus on events that may occur at any time from pregnancy to early childhood, depending on individual circumstances. In order to present the information in a logical manner, we have divided the concept into the following four major sections, with a listing of each data source relevant in each section. 1. Prenatal Data Sources

Two of the most relevant data sources related to the prenatal period are:


• Medical Services Data Description - contains service claims for physician visits in offices, hospitals and outpatient departments; fee-for-service components for tests such as lab and x-ray procedures performed in offices and hospitals; payments for on-call agreements (e.g. anaesthetists) that are not attributed to individual patients; as well as information about a physicians' specialty.

  • diagnosis – Note: only one diagnosis is recorded per claim / visit

• Midwifery Summary Reports Data Description - contains administrative health data from the Midwifery Program in Manitoba concerning all women that receive prenatal, birth and postpartum care from a midwife.

• Healthy Baby Community Support Program Data Description - contains information about women who participate in social support and informal learning group programs available to all women from the prenatal period through to an infant's first birthday. These programs aim to encourage early, regular, prenatal care and promote healthy infant development by building women's confidence and awareness and offering healthy eating through cooking and nutrition activities.

• Healthy Baby Prenatal Benefit Program Data Description - contains information about participation in a program that provides financial benefits to help women meet nutritional needs during pregnancy and acts as a mechanism for connecting women to health and community resources in their area. Data are available for all women who have applied for the benefit as well as the applicant's spouse (approximately 1/3 of women giving birth in Manitoba receive the benefit).

• Hospital Abstracts Data Description - contains individual hospital episode of care records completed after discharge that contain demographic, clinical and service information.

  Relevant hospital abstracts data in the prenatal period may be related to complications or conditions of the mother or fetus prior to birth.

• Families First Screening / BabyFirst Screening Data Description - contains survey data regarding the supports and challenges that families experience, including biological, social and demographic risk factors that may be present during the prenatal period. Screening information is captured for all families that have agreed to participate in the program. Data are collected within a week of a newborn's discharge from the hospital by Public Health nurses using the Families First / BabyFirst screening form.

• Insight Mentoring Program Data - contains information about women who participate in a program aimed at decreasing the number of children born with fetal alcohol spectrum disorder (FASD). The program is for women aged 18 years or older, have substance use problems, are pregnant (or have recently given birth and used alcohol or drugs during their pregnancy), and are not well-connected to community resources.

  Data Highlights:

• Manitoba Maternal Serum Screening Program Data Description - contains clinical health data concerning the prenatal genetic screens of approximately 65% of Manitoban pregnancies each year.

2. Birth Data Sources

Two data sources provide information directly related to the birth of a child:


• Hospital Abstracts Data Description - contains individual hospital episode of care records completed after discharge that contain demographic, clinical and service information.

  For a birth, two different abstracts are created; a maternal abstract and a newborn abstract, that collect additional data related to the birth. The maternal abstract contains:
  

  • the outcome of delivery
    
  • gestational age
    
  • number of previous term deliveries, gravida and parity
    
  • prenatal visits (e.g.: count of visits, date of first prenatal visit)
    
  • smoking and alcohol use during pregnancy
    
  • newborn weight
    
  • number of previous spontaneous / therapeutic abortions

• Manitoba Fetal Alcohol Spectrum Disorder Data Description - contains clinical health data on services provided to Manitoba preschool children, school age children, adolescents, and adults with Fetal Alcohol Spectrum Disorder (FASD) identified through the Manitoba FASD Centre program.

• Diabetes Education Resource for Children and Adolescents (DERCA) Data Description - contains clinical health data for children diagnosed with diabetes in Manitoba. All children diagnosed with youth onset Type 1 Diabetes and Type 2 Diabetes, since January 1986 are included.

Social Data - Postnatal to Early Childhood

• BabyFirst Evaluation Data Description - contains data collected as part of a provincial assessment of the BabyFirst home visiting program conducted by Healthy Child Manitoba (HCM) - services now delivered by Manitoba Child and Youth programs. This evaluation focused on effects of the BabyFirst home visiting program (a targeted, multiyear, provincial home visiting program for families with newborns living under conditions of risk) on parent and parent-child outcomes.

  Data Highlights:

  • social, economic, and health-related outcomes for all families with newborns who were determined to be eligible for the program.

  
  Note: The BabyFirst Evaluation Program data consists of 250 records between January 1, 2000 and March 31, 2003. The Families First Program replaced the BabyFirst program in 2003.

• Child Day Care Program Data Description - contains information on children enrolled in provincially licensed child care facilities whose families paid a subsidized rate (about 12, 500 out of 25, 000 spaces).

• Early Development Instrument (EDI) Data Description - contains data used to assess kindergarten children's developmental health and a child's readiness to learn in school. The assessment covers five domains:

• Cadham Provincial Laboratory (CPL) Data Description - contains public health laboratory services and reference services for the identification and typing of microorganisms.

• Shared Health Diagnostic Services (SHDS) - currently named Diagnostic Services Manitoba (DSM) - Data Description - contains information on laboratory and diagnostic imaging services for Manitobans. This includes chemistry, immunology, hematology and microbiology lab tests.

• Emergency Care – Admission Discharge and Transfer (ADT) and E-Triage Data Description - contains information on visits to the Emergency Department (ED) at hospitals in Winnipeg.

  Note: The ADT E-Triage data is available from 1999/2000 to 2010/2011. After this date, emergency care data is captured in the Emergency Department Information System (EDIS) (see data description below).

• Emergency Department Information System (EDIS) Data Description - contains data about a patient's visit to the emergency department, from first point of entry at the triage desk through to discharge.

• Manitoba Cancer Registry and Treatment Data Description - contains a registry of all Manitobans diagnosed with cancer and the treatment they received.

• Manitoba Health Insurance Registry Data Description - contains a longitudinal population-based registry of all individuals who have been registered with Manitoba Health at some point since 1970. It provides historical information for follow-up to track residents for longitudinal and intergenerational analyses. MCHP receives a copy of the registry data semi-annually (twice a year) from Manitoba Health.

• Medicine Data Description - contains episode of care information for individuals admitted to a Medicine ward in any of four Winnipeg hospitals.

• Mental Health Management Information System (MHMIS) Data Description - contains comprehensive case management information for Manitoba residents who receive clinical, social, or rehabilitation services for mental health.

• Canadian Census Data Description - contains survey data based on the Manitoba population, which includes individual level and aggregate level demographic information for persons within a dissemination area (400-700 people).

• Social Allowances Management Information System (SAMIN) Data Description - contains information on Manitoba residents who receive support from the Employment and Income Assistance (EIA) Program, a provincial program of last resort for people who need help to meet basic personal and family needs.

This data does not include the outcomes of charges.

Concepts Related to Maternal / Child Health

MCHP has developed several indicators that measure aspects related to maternal and child health and are documented in the online Concept Dictionary. There are four specific concepts available that provide an overview of these indicators and links to the detailed methodological information contained in the Concept Dictionary. The concepts are identified and described below, along with a hyperlink to each concept.

1. Maternal and Newborn Health Status Indicators

This concept identifies and lists the maternal and newborn health status indicators reported in the Perinatal Services and Outcomes in Manitoba deliverable by Heaman et al. (2012). The concept lists the indicators, and provides links to the corresponding definition or concept and to the related methods and discussion sections that are documented in the deliverable. The five categories of indicators are listed below, along with some of the indicators included in each category. For more complete information, see the Maternal and Newborn Health Status Indicators concept.


1. Profile of Women Giving Birth - women giving birth with less than a Grade 12 education; women giving birth receiving income assistance; live births to teen mothers; and live births to women aged 35 and older.
   
2. Maternal Prenatal Health - alcohol consumption during pregnancy; smoking during pregnancy; use of infertility drugs prior to live birth; and antenatal hospitalizations.
   
3. Giving Birth - travelling to give birth – by distance or outside of RHA; induction of labour; breech births; and severe maternal morbidities.
   
4. Maternal Postpartum Health - breastfeeding initiation; postpartum hospital readmission; and postpartum psychological distress.
   
5. Fetal and Newborn Health - stillbirths; small / large for gestational age; five-minute APGAR score of 7 or less; and neonatal Special Care Unit (SCU) admission.

2. Child Health Indicators

There are three concepts available that provide detailed information on the child health indicators used in MCHP research over time. These concepts reflect different time periods and contain similar information / indicators. Each concept contains hyperlinks to more detailed information in the MCHP Concept Dictionary / Glossary, and to the corresponding information in each research project.

For example, the concept Child Health Indicators (2008) lists all of the child health indicators presented in the Manitoba Child Health Atlas Update deliverable by Brownell et al. (2008). The nine categories of indicators used in this report are listed below, along with some of the indicators included in each category. For more complete information, see the Child Health Indicators (2008) concept.


1. Perinatal Health - teen pregnancy; caesarean sections (C-section); breastfeeding initiation; and newborn readmissions.
   
2. Health Care Utilization - childhood immunizations; injury hospitalizations; physician visits; and continuity of care.
   
3. Use of Prescription Medications - any prescription; antibiotics; antidepressants; and psycho-stimulants.
   
4. Infant and Child Mortality - infant mortality; stillbirths; child mortality; and injury mortality.
   
5. Education - retention (continuing students); school mobility; and high school completion.
   
6. Nutrition and Physical Activity - fruit and vegetable consumption; physical activity; and exposure to second-hand smoke.
   
7. Childhood Chronic Conditions - asthma; diabetes; and autism spectrum disorders.
   
8. Community and Social Services - licensed child care spaces; receipt of income assistance (IA); and children in care.
   
9. Adolescent Health and Reproductive Health - adolescent smoking; condom use; and birth control pill use

The other two concept are:


• Child Health Status Indicators (2001) based on the information in the MCHP Deliverable Assessing the Health of Children in Manitoba: A Population-Based Study (2001), and
  
• Child Health Indicators (2012) based on the information in the MCHP Deliverable How Are Manitoba's Children Doing? (2012).
  
  These two concept list some of the same indicators used in the 2008 Deliverable, but are organized differently than the 2008 information.

The following cautions / limitations apply to this concept:


• this concept does not contain a detailed listing of all the variables that are available in each of the databases. The concept is intended to provide a summary of all the data sources related to maternal / child health and early childhood, and includes some of the relevant variables available in each data source. This includes data sources from the health, social, justice and educational areas of the MCHP Data Repository. For more information on each of the data sources, please see the Data Descriptions available on our web site.
  
  
• although some of the earliest data in the MCHP Repository dates back to 1970, NOT ALL the data listed in this concept is available that far back, or for current years. Please see the Data Years Chart on our external web site to identify what years of data are currently available for each of the data sets.
  
  
• the work on this concept was undertaken in September 2015 to July 2016 and the content in this concept is subject to change.
